This section takes you through the process of getting an assessment, finding appropriate programs and agencies in the mental health and developmental services sectors and developing a support plan.

Introduction
If you have arrived at this step along the path, you will have information that leads you to believe that:
- the person that you are helping has a developmental disability and a mental health problem, and
- there is something missing in the person's housing situation and/or in the supports he or she needs to find and keep housing.
This section is about organizing the information and filling in the gaps. It talks about:
- where to get a formal assessment that will provide an accurate picture of the person and his or her needs
- how to navigate through the developmental services and mental health sectors and connect with an agency that can help you access and co-ordinate the services that the person needs
- how to develop a formal support plan and a plan to help prevent and manage a crisis.
