2008 Application and Review Process

All information should be typed into the application form using the spaces provided with 12pt text (Times). Attachments will not be accepted in response to questions on this form.

Attached to this application form are four documents for your reference. They are:

  • Attachment A Special Requirements form
  • Attachment B Confirmation of Agreement to Supervise
  • Attachment C Referee Rating Form

Application Checklist  

Send one package that includes:

  1. One original application package: the completed application package includes the application form and supplemental pages and documents. Please do not staple or bind the pages of your original copy. Use a binder clip to secure the full package of originals. For Attachments: Please ensure that your first and last names appear in the upper left corner of each page. Originals should be assembled in the following order:
    For Graduate Students/Fellows For Investigaters
    a.       application form;
    b.     curriculum vitae/resume ; b.         CIHR common cv;
    c.     one example of scholarly writing completed by the applicant
    d.     Special Requirements form (see Attachment A);
    e.     Confirmation of Agreement to Supervise (see Attachment B);
    f.     an official copy of all university transcripts in
    sealed envelopes;
    f.      an official/notorized copy of all
    postsecondary degrees/diplomas;
    g.     three Referee Rating forms (See Attachment C – each sealed in an envelope with the referee’s signature across the seal); and
    h.     a 3.5 inch floppy disk or CD with electronic copies of the first 3 items in Microsoft Word format (documents will be accepted via email).
  2. Eleven collated, double-sided photocopies of the application package. Photocopies may be stapled or bound. Use a binder clip to secure the each copy. Please ensure that your first and last names appear in the upper left corner of each page. Photocopies should be assembled in the following order:

    1. application form;
    2. curriculum vitae/resume;
    3. one example of scholarly writing completed by the applicant;
    4. Special Requirements form (see Attachment A); and
    5. Confirmation of Agreement to Supervise (see Attachment B).

Applications will be reviewed by a panel using the criteria outlined in Attachment D.
