Sample Letter—Acute Pain Management during Methadone Maintenance Treatment

Sample Letter—Reimbursement for Transportation

This is a template for a letter that can be used for clients in the Ontario Works program to recoup the cost of transportation (e.g., bus fare or transit pass) to pharmacy or clinic for methadone treatment. It should be customized for each client, including the number of days a week the client needs to travel to obtain methadone doses. (Blanks to be filled in are denoted with <<   >>.) **Note that for Ontario Disability Support Plan (ODSP) clients, physicians can bill OHIP for filling out a transportation form, which the client must obtain from ODSP directly.
