Clinic Toolkit for Methadone and Buprenorphine Providers
This toolkit comprises a number of documents frequently used in the treatment of opioid dependence in an outpatient setting
in Ontario. It was compiled by the Centre for Addiction and Mental Health (CAMH) as part of the OpiATE Project, an initiative
funded by the Ministry of Health and Long Term Care with the objective of enhancing training and professional supports for
methadone providers. The toolkit includes various forms, letter templates and quick references that will be useful to practitioners
involved in methadone and/or buprenorphine maintenance treatment.
The toolkit components come from a number of sources: some were developed at CAMH's Addiction Medicine Clinic or by individual
physicians who practise at this clinic; others originated with the College of Physicians and Surgeons of Ontario (CPSO), the
Ministry of Health and Long Term Care (MOHLTC) and the American Society for Addiction Medicine (ASAM). This toolkit collects
these documents in one place for your convenience.
Some of the document files are provided in Word format so that they can be easily adapted, e.g., by adding your clinic contact
information, inserting a logo, and/or editing content to suit specific circumstances. Other components are provided in PDF
format since their content is fixed. These include some forms with input fields that can be filled in onscreen using Adobe
software; only the signatures need to be added after printing.
