What is ADAT?

Admission and Discharge Criteria and Assessment Tools (ADAT) is a comprehensive approach to establishing an initial treatment plan which identifies the most appropriate level and intensity of care for a client entering Ontario's addictions treatment system (or whether the client is ready for discharge). The Admission criteria in particular use the results of the Assessment Tools to this end. The Assessment Tools component is a suite of clinical assessment tools to be used by professionals to identify a client's strengths and needs in seven areas. The Ontario Ministry of Health and Long-Term Care (MoHLTC)-Addiction Program has mandated the tools and criteria for use by for all ministry-funded addiction service programs. CAMH holds copyright to six of the eight tools that are in ADAT and to the ADAT standardized manual (Assessment, Discharge And Treatment: Standardized Tools and Criteria Manual).

Additional form to complete: The Referral and Decision Tracking Summary

  • mandatory form to record all of the final scores for the tools that are scored (SOCRATES, TEQ, DTCQ-8, BASIS-32, PSS) as well as the relevant comments for the remaining tools (DHQ, AC, HSF)
  • helpful for summarizing the results of the tools in one place
  • can be used as a face sheet in a client file as well as a referral form that can be sent (with appropriate client consent) to potential referral destinations.
