The eight core assessment tools should be thought of as a minimum data set. The information they gather would not be the only information necessary to develop a treatment plan.

The Clinical Profile Form (also called the Client Profile Sheet) can help the clinician to note other relevant information about the client -- to examine each of the areas of strength and need, and additional information collected during the assessment process-to generate a clinical profile of the client. It can be used to summarize assessment information for the client's file and can also be used when sharing information with referral destinations.

This optional form can be used if your agency does not have a similar form or if this form is more useful.
