measures mental health status in five major areas:
- relation to self/others
- daily living/role functioning
- depression/anxiety
- impulsive/addictive behaviour
- psychosis
determines if daily living skills and/or mental health functioning would likely interfere with treatment for substance use
measures functioning only over the past week
can be self-administered by client following instructions
