The Shared Care Clinical Outreach Program is a partnership between CAMH, the University Health Network and St. Michael's Hospital.
The team works with men and women who are homeless and experiencing serious and persistent mental health concerns. The Shared
Care Clinical Outreach Service addresses the physical and mental health needs of homeless people using the shelters, hostels
and drop-ins in the downtown Toronto area.
Team members include full-time registered nurses, outreach workers, a housing worker, visiting family physicians and visiting
psychiatrists as consultants.
Shared Care teams work collaboratively with staff from other agencies to deliver respectful and comprehensive care and facilitate
access to resources, enhancing the individual's quality of life. Teams are located at hostels and community centres throughout
the city.
