The Shared Care Clinical Outreach Program

This program, in partnership the University Health Network works with men and women who are homeless, and experiencing serious and persistent mental health concerns. Shared Care Clinical Outreach Service addresses the physical and mental health needs of homeless people using the shelters, hostels and drop-ins in the downtown Toronto area. Team members include full-time registered nurses, outreach workers, visiting family physicians and visiting psychiatrists as consultants.
