Community Treatment Order (CTO) Coordination

CTO Coordinators provide assistance to physicians and inpatient hospital teams to coordinate the process and development of community treatment orders (CTOs).  The Coordinators are located in mental health units at various Toronto hospitals and provide consultation and support to assist in the various aspects involved in the development of a CTO.  Coordinators work with all parties involved in a CTO to ensure proper process is followed, including completing appropriate forms and documentation, accessing right’s advice, and facilitating the development of a comprehensive community treatment plan that addresses an individual’s treatment and support needs when they are discharged from an inpatient hospital setting.  This may include making referrals for ongoing support services as part of the community treatment plan.  Individuals eligible for this service would need to meet criteria for a CTO as detailed in the Mental Health Act.
Hours of Operation:  Monday to Friday – 8:30 a.m. to 4:30 p.m.
