OHIP Clinic

Provides clients and staff with an opportunity to obtain or replace OHIP cards. Cards may need to be replaced if they have been lost, stolen or damaged or the address has changed; new ones may be necessary for a new resident of Ontario. Three pieces of ID must be brought to the clinic.
